What are the fees for Intensives and single sessions?

A mandatory $1000.00 deposit is required at time of booking to reserve
your scheduled Intensive, regardless of private pay or insurance carrying
patients.
Multiple Intensive Discounts will apply during one calendar year
$1000.00 deposit is required to reserve each Intensive scheduled
2 Intensives – 5% discount $8265 ea.
3 Intensives – 7% discount $8091 ea.
4 Intensives – 10% discount $7830 ea.

What is the Deposit and Cancellation policy?

When you are booking an Intensive session, we require a $1000.00 deposit which will go towards your session.
If your insurance is covering your costs, any remaining deposit will be refunded after your session is complete, has gone through the insurance underwriting process, and any verified sick or emergency days are accounted for.

Before an Intensive starts, deposits are non-refundable. However, a portion may be credited to a later Intensive date according to the conditions below.

60 days prior to the Intensive start date you may transfer your deposit to another Intensive session but will be charged a $100.00 cancellation fee.

30 days prior to the Intensive start date you may transfer your deposit to another Intensive session but will be charged a $500.00 cancellation fee.

0-30 days prior to the Intensive start date we are unable to transfer or refund any amount of your deposit; however, if you have made any other payments towards your Intensive, that amount can be transferred to another Intensive session.

$40.00 will be charged for No Shows or same day cancellation
$25.00 will be charged if you are more than 15 minutes late

When are payments due?

Deposits are due at time of registration to hold Intensive placement.
Self-Pay Intensive balances are due 30 days before the Intensive session begins.
Payments for single visits are due on the day of service.

Payments include private pay fees, co-payments, unmet deductibles, late or
cancellation fees and any other miscellaneous charges.
Services not paid for at the time treatment was rendered will be billed to the patient’s
parent/guardian.
Payment of any outstanding amounts will be due within 30 days of billing.
Any unpaid balances of more than 60 days will be assessed a 10% late fee.

What Insurances do you take?

We take many major Insurance plans including Medicaid, however there are some we do not take– please contact us for more information

How do I know if I am covered?

As soon as we get your information from your intake form, we will verify your coverage with your insurance company and determine benefits. This will not be a guarantee that your provider will pay the full amount of your sessions. You are ultimately responsible to pay for any amount that may not be covered by your insurance company including co-pays, co- insurance, or any other fees.

We encourage you to call your insurance company to determine if we are in network with your policy and the exact benefits your plan provides. We can provide you with our billing codes if they are needed for approval. (Billing codes we use during treatment: 97163, 97164, 97110, 97112, 97116, 97140, 97530.)

What are my insurance plan requirements and benefits?

We encourage you to contact your insurance company directly to determine your exact benefits; however, these are some of the common requirements and benefits:

Referrals from primary care physician
Co-pay or co-insurance at the time of service
Preauthorization for initial or more visits
How many hours per session of therapy you are eligible for
Limits on visits for each calendar year
              Hard Max. You are limited to a certain number of visits in the calendar year and
you cannot appeal or request more.
             Soft Max. You can apply for more visits when you have used your allowed visits.
Your insurance company will review and determine if you are eligible for more.
At times insurance payments can be delayed, changed, or denied. If this happens, we
would appreciate your help in resolving any issue.

What if you are not a provider for my insurance?

We are providers for many insurance carriers; however, there are some that we are out of network with. Options in this case are for you to research and utilize your out-of-network benefits; contact your insurance company to see if they will cover you; or self-pay.

What are other options for those that are Out of Network?

You may elect to self-pay and submit a super bill to your insurance provider for reimbursement after the Intensive session is complete. Let us know ahead of time if you will need a super bill. Find available grants for which you can apply (e.g., UCCF) Go Fund Me. We also take Care Credit
